Students requesting letters of recommendation
(also known as reference letters) from faculty
and staff on campus should provide these
individuals with a
Recommendation/Evaluation Authorization and
Waiver
form.
This will allow the faculty/staff member to
address specifics regarding your academic record
(i.e., grades, quality of work in particular
courses, etc.).
